According to Decree of the Ministry of Health No. 467. Prices are valid from 1.4.2022.

*) A GP can issue an employee's health condition assessment only in risk category 1 (as issued by Public Health Office). If you apply for work that has been issued category 2 or above, you need medical record extract for the contracted health center.
